Question about Shimano compatible cassette sprocket and spacer thickness for a given speed

Well the hub axle measures 145mm across

And the drive side is 6mm of thread
And off side 5mm

Problem is when I loosen the drive size locknut even by 1mm the axle develops play
May be an issue with the hub

I guess a spacer behind the locknut of say 2mm and 1mm on off side could solve this ? Seeing as the drive side has 1mm extra
 
On the plus side the steel lockring is a full 1mm thinner
But on the minus side it’s too narrow to secure the smallest (silver) cog

Sorry to say this but you're measuring the axle [slightly] the wrong way. You need to measure the overlock nut dimension, i.e. outer distance between nuts. We can deduce this by taking 145 - 5 - 6mm = 134mm but maybe not quite as accurate as it's three measurements taken. That dimension really needs to be 135mm. Looks like you are 1mm out. Yes, a washer behind the nut should solve it.

On another note, the last position cog is a mile from the spokes... but I don't think/see there's a way to easily change that on your freehub (without grinding something).
