quill stem wedge woes

it's not a big frame and I'm only 5'10''
150mm is the length of the actuall bolt, how are the stems measured? from the 'stop' line to the bars? I know very little about old bikes, sorry.

Just the bit that sticks out the front.

In other words,

measure it from the bolt that tightens the quill along the stem to the centre of the handlebars

Wedges are in all different angles, be prepared of grinding the stem to match the new wedge.
