Been playing/adjusting an old FSA Campagnolo compatable hiddenset headset that was original on the cannondale caad9, i built this winter. It seems it has seen better days.
A NOS Chorus hiddenset set came up on Ebay from one of my favorite seller - goes by the name of 'house of cogs. He has a Lot of hard to find NOS btw. worth a look

Headset seems to loosen on rough Surfaces/Pavement . 5nm bolt recommended setting seems to be the obvious culprit i am quessing. Hopefully

always a conundrum with bike parts imo until there isn't

That's for a threadless steerer-

In some instances of old/early ahead stem/ steerer interface, the inside of the stem takes a "print" off the steerer clamping area, so when it's tightened down, the last fraction of a mm, it pops back into the groove it had already made.

I haven't seen this for a long time, but might be your problem.

If that's the case, try a slightly different stem, or change a spacer by 1mm to move the 2 sections away from the "historical" bond.
