Need help to identify the frameset

This frame is a puzzle to me. It has no name so far but I believe it is something special. Dropouts are campagnolo. Please take a look at fork crown! It is thin and rounded towards head tube. I have never seen such a fork before. All the lugs have similar motive. Shifters pivots are placed asimmetrically, right is a bit higher than left.
