I've done the job "properly" many times, both road and mtb, here was one of my best efforts. Bob Jacksons' brazed on the IS disc mount and did all the work, and a total cable re routing with hose tabs and removed canti bosses, rode like a dream. To do the job isn't a cheap option though, work was over £300 a few years back, then good top end parts, was well into four figures.
