shouldn't it come with a shim for the stem clamp?

You get one if you buy a new one, and need one to fit a 31.8mm stem. They're 1in tubing in the middle, so should go straight into a 25.4mm stem. Although Jeff seems to recommend the shim/oversize stem combination.

I rode the titec versions the other week, they are the same shape but in 6061 Alu. They looked weird but as soon as I sat on the bike they fell easily to hand and felt very natural and comfortable. I was well impressed after just a short 15 to 20 min ride and am getting a pair for one of my old Stumpies. I like these Ti ones but cant afford them.

Thanks,I was thinking when you hold your arms out your hands naturally seem to fall to that position,might check out the Titec ones,not fussed about the material
