You removed the little slim steel washer?

That stops removal, killed many crank extractor threads

What stops the bolt from pushing out

It's screwed into the opposite end of the bottom bracket spindle.

So the bolt needs to be the same pitch as the bb thread an it screws all the through
My bb thread is approx m12 but I can't get anything bigger than an m8 bolt through the length of the axle as there is a step inside it
