Depends on the cantilever I think, though I've never bothered with those and just used a hanger!

None of them. A hanger and straddle cable work far better in all ways except that if the main cable breaks the stable can catch on the tyre but how many brake cables have you broken in your life?

A mate of mine had that happen to him some years ago going down a steep hill on an MTB with nobbly tyres. He lost his ear when he hit the deck so I started to fit a long bolt and a couple of nuts through the hole in the crown - just in case!

The fork mounted front reflector bracket was usually a safety device for this situation:
