It would be more wise to have a sleeve inside the steerer to prevent the threaded portion from collapsing or sheering off.

Sounds like a good suggestion, but I'd have to remove the star nut, then I'm not sure whether it would be more difficult to put back in, depending on how thick the sleeve is. I'll look into it, but the cost of having a sleeve machined would probably be prohibitive.

I'm probably thinking 50% about the integrity, though it is unlikely it will get stressed even as much as it has previously, and 50% about avoiding damage from the threads to the inside of the stem.

I've done it, and I'm still here, but I'd avoid doing it again if at all possible, for all the reasons already mentioned. It would be easier to sell the Manitou fork and get something else in Ahead flavour, these old bikes and parts are failure prone enough without adding another element into the mix

I'm running a threaded fork and an ahead stem on my commuter. Has a long old Kore 130mm stem, steel steerer. Daily rider, been setup that way for close on 15 years now, no issues. Fork is a spinner.

That is very similar to my setup (Kore stem too). Have you done anything to prevent the threads damaging the inside of the stem?

Nope. Just running as is, wasn't too concerned about damaging the stem, but it's held up fine with no visible damage.
