On a Campag lever, I drilled small cable sized hole in the bottom of the inside rear at an angle through to a few millimetres above the handlebar. I then counterbored with a larger drill from the outside a short way as a stop for the outer cable. The cable route is not a lot different from normal and they seem to work OK.
