Quill stem in a threadless fork?

No, you'll still need a stern clamped to the outside of the steerer for it to tension against

I don't think so. Isn't that just an adjustable spacer? You'd have no real way to tension the headset bearings unless you are considering trying to use the adjustment to force up against the stem. I don't think it will work or a good idea personally.

Also, what would stop the spacer unthreading? Can't tell from the pic how that would work even using an ahead stem.
