Thanks for the comments and suggestions folks.
A rear mech cable adjuster does fit this clamp (M5) so that may be a workable alternative to drilling your own.
I finally got around to taking the scallop out on the miller so that this sits lower down the seat tube. Not a great job to be honest, needed a slightly larger cutter in hindsight but that's it for now. This project is on the back burner for a while as others have jumped the queue so good luck to anybody that tries this for themselves. Let me know if it works ok or if I've totally wasted your time
