I have a 125mm brown Velocity stem that unfortunately has the point of the quill sheared off. Stupid mechanic put it back in way above the min insertion point after the last service, and stupid me didn't notice.
It can still be used safely as long as the stem is inserted right down into the steerer as far as it will go - I used it like this 'til I got a replacement. I also have the sheared part if you want to try to repair it.
