Formula older front disc I.D ?

Hi ,have recently bought a Formula front disc brake for my single speed retro build and find it won't mount cleanly to the Bontrager Race rigid forks.The disc unit sits to close to the fork leg and it is approx half a hole out for the bolts to fit thru.The disc has no model number I can see ,its just silver alloy with red print saying Formula ITALY .
Can I somewhere to ID it and how do I mount it.Do I need a adapter ? or should it go straight on .Using a 160mm rotor.Are there any websites that list models of Formula brakes to see if I can see mine .thanks
 
Formula used 49mm not 51mm hole centres, it's also not the right distance from ISO hubs for the disc to fit.

I doubt you'll find an adaptor to mount Formula to ISO. If you use early Marzocchi Bombers they have Formula mounts as standard, you only then need a Formula hub to make it work.

Not impossible, just very close to it.
