I was thinking custom, but it looks a bit like a Ti Syncros Cattlehead, though i can't say one was ever made. It would've gone nicely with their syncros Ti seat post. It appears this is the setup. Might want to find out if Syncros made a Ti cattlehead, even in very limited production.

*it also has some of the same of the Titec Ti stem by the handle bar clamp, but the steerer clamp would be wrong for that stem, unless, again, it's an prototype. I know the Ti Fats were spec'd with the titec ti stem at some point, and they were also spec'd with syncros parts, so it's probably from one of these companies. What do the fatcog folks say?
 
There was a similar stem discussed here a while back. The work sure looks similar to that on the One-Off I used to have.

thought One-off only built ti frames, but I guess if you can build a Ti frame (that's an understatement for One-off), then i guess a stem wouldn't be too hard.
 
sweet, but what makes you think this stem now, is a one-off? The welds look cleaner on your old One-off, and the shape/clamps are all different.

I'm probably wrong, but I'm going with a Syncros/Titec proto or something to that effect. There's nothing else on your new ti fat that would suggest he was up on that high end, low production, handmade stuff from back then. Then again, it could've came on the bike. He added the generic cable stop, hmmmm.
