I think much of the time its just intolerances of the material in the headtube :? The cup going into it is machined to a tight tolerance and the internal section of the headtube is not. I've found an alloy tape take up these differences for a snug fit.
Cannot for the life of me do i know what its called only its extremely thin with a peel off glue backing.
When you remove the headset cut you can see on it where the foil has stuck and where its scraped off, that to me indicates intolerances in the interface which can id suspect always be the headtube, unless of course that has been machined internally.
