Sounds like you are potentially using the steel bar wedges rather than the alloy bar wedges. Bullets came with two sets in the box from new and you swapped them according to whether you had narrow thickness or wide. Alternatively you may have one wedge still compressed in there. Loosen it off and a swift tap to see if that frees it. When fitting make sure you push the Allen key into the end of the shifter and basically install the shifter on the bar by pushing it on with the Allen key as that keeps the wedges from compressing.
With regards the limited number of positions make sure you’ve run the cable around the whole shifter body before it exits.
