50mm chainline is OK.

Did you check the distance between the derailer and the big chain ring vertically? The smaller the distance, the better.
What kind of chain do you use?
Does the derailer match the chain ring size?

Usually the chain will only fall off the big chain, if you overshift it. That would be solved by reducing the upper limit stop.

Thx... the problem is, when I reduce the upper limit stop to much, I can not shift up anymore.

I have this Chain CN-HG91 XTR
 
The Last days I tried with a mate everything. No chance... We could move the BB abit to the right. Now I have a Chainline of 51mm... Same problem.
 
Hey guys... No chance with my 107mm...I tried everything

Possible that I really need a 113mm for my XTR 900 Chainset
