You need to take into account the overhang of the seat tube as compared to the top tube :)

You actually don't, that's just style, it doesn't alter the frame size.
This will still have a 23" toptube, the ~3" drop doesn't make it shrink to a 15-16" frame with a 21-22" toptube.
 
That depends on the exact frame geometry used by a certain manufacturer.

If they started off with regular proportions and just made the top tube more sloping, while keeping the rest of the frame the same, then the actual size will correspond to the c-t measurement.

However, if they just added some extra tube on top of a seat tube of a small frame, just for unique looks, then the rest of the frame will still be small indeed.

That's why the seat tube length is quite irrelevant on it's own really.
 
For clarity, at this time, Kona frame size measurements are the total length of the seat-tube, in inches. It's an 18" frame, and that's the reference many will use to understand it's size and potential fit.
