Price includes me posting this 1st Class Recorded. This has a titanium clamp bolt as seen in the pic, not the original steel Shimano one. And yes it also comes with the cable adjuster wheel thingy.

I have never tried though I am sure a few have used such combinations?

I believe it would, all Shimano rear mechs work in a similar manner.
