I've got one somewhere which is based on information from my SMBLA Training Manual.

not sure what ensues with the specifics of a mountain bike one but when at college we used a risk calculation matrix annd a pretty simple one off the net i think
was a case of common sense, walk around the workshop and look for potential hazards (in this case, track, obviously there are other things to be considered)
