octalink BB, wrong threaded sleeve?

just put an octalink BB in and the N/D/S just kept going till it hit the frame.
Seems the distance between the flanges is 66mm, so I am either going to turn the flange off or make a 2.5mm spacer. I have to assume I have been given the wrong part? The loose sleeve is marked 68mm.

Any one else come across this?

DS locates the bottom bracket across the frame and concentricity within the shell (chain line etc)
NDS is just for concentricity, iirc there should be some sort of white plastic ring embedded inside the NDS cup to locate the cartridge against its ID. Probably ptfe or similar. It doesn't need to butt up against the cartridge.
Tolerance band on a 68 mm shell is around 1,2mm from what I can remember.

All turns to crap if the cartridge comes adrift from the DS cup!
