Replacing quill stem question

If the current quill is 22.2mm and fits into the steerer
and the shim part is 1 1/8 but sits on top of the headset.
how would a 1 1/8 quill fit? is the actual bit that goes inside the steerer smaller than 1 1/8?
or is it forced into the steerer?

The point about the stem having to be a 25.4 to fit a 1 1/8 steerer is that classically the steerer walls were 1/16th thick, so you need a 1" tube to fit inside the 1 1/16" one.

So do I order a new 1 1/8" quill threaded stem? or 1"?

I apologise - if you order a 1 1/8 stem, they will send you one with 25.4 stamped on it. That's because what we call a 1 1/8 stem is a stem for a 1 1/8 system, i.e., one that measures 1". Similarly your stem with 22.2 stamped on it (7/8") is called a one inch stem. Don't worry, it always causes confusion!
