I used a screwdriver to lever up on the bolt as I undid it (it pulls the inside bit up against the tube so it doesn't spin as you unscrew).

Place magnets on around the outside edge of the tube to catch and hold all the bits, gently rotate the magnet/s to the open hole, any such remnants will follow the magnet.

Will save you having to remove the chainset & bottom bracket.
 
Yes you would. But you can shake them out through the hole that is inside the steering tube. You will will have to slide out the forks for that.

I had a very similar problems on my Klein Pinnacle. I used a dremel tool to grind down the sides of the rivet until I was able to press it through the hole into the frame. Quite delicate work, that.



The remaints of the rivnut did not want to shake out of that little hole:


So I put a dab of grease onto an old spoke and went fishing:
 
I can imagine. Maybe a magnet would work if the frame is aluminum (which is my case).

I don't have a dremel so I'll have to drill ...
