Have you tried cleaning the internals, I have had many that looks like a total lost cause, all have come good after a good clean.
The problem is the old Shimano grease, it goes hard and the lever seem broken. Fires through, no ratchet, etc etc.
Remove the shifter from the brake lever and the bottom cover, then soak it overnight in white spirit, try to operate it occasionally when soaking. I bed you find that it now works. Shake the white spirit out and lube with something like GT85.

It keeps sticking and then it will move without moving the derailleur.
Did take the cover off and couldn't see anything loose inside.
Once you get it to change gear,
Changing into smaller cogs at the rear is fine, it just appears to be changing into the larger cogs at the rear.
I honestly think it's had it.

From your description of the problem, they are typical of the grease going off.
Go on give it a good soaking, what have you got to lose.
