Richard, yours may be well after my time, but the Campagnolo braze on levers I remember had a flat alloy washer on first, then the lever with the concave side outwards, then the flat spring steel washer. and the chrome outer washer with the convex side inwards, the pressure of the screw would slightly deform the flat spring washer.
If there was any movement of the flatted washers on the boss then they would loosen. we used to have part no 604/1, a chrome screw with a small loop on it to act as a wing nut.
Just a thought, they could be on the wrong sides, with the concave part of the lever inwards.

Keith,

Thanks for that, i will check them before i change them but i think they may be fitted right and 28 years ago they came loose and i changed the levers to ones which did not. I have the hand tighteners so i can swap them easily enough - and, the added benefit is they look nicer!
