Hex bolts with washers attached?

Anyone know where I can get hex bolts (for standard frame braze-ons, rack-mounts, etc) with washers attached? You find them on some stem face-plates and other accessories.

I'm fed up of my rack bolts shaking loose and dropping out, and if I use separate split-ring washers they get lost when I take the bolts out. As I take the rack off regularly I don't want to use any kind of glue either.

Wouldn't the o rings just get squashed and mangled and drop off?

I know these things exist because I've seen them - they are called "captive washers" apparently, but still no nearer sourcing them.
 
Don't think the captive washer types used on stems etc are for shakeproof ness if its the same ones I'm thinking of. Try the serrated type if you can get to a supplier you could get a imperial/metric sizing that screws on.
