i did the inboard bar end thing years ago when i was getting over a pair of dislocated shoulders as it was the only way i could ride any sort of distance without being in excruciating pain (needed to keep my elbows wrists & shoulders in a straight line.)

it made braking interesting (turned the levers around) & shifting was even more ropey! ran out of space inside the bar ends so slid the thumbies underneath like a poor mans trigger shifter!
